Enrollment Documents

To help maintain compliance with the Maryland Department of Education's Office of Child Care licensing regulations and Maryland EXCELS Quality Rating System, the following documents must be fully completed and returned before your child may attend.  Incomplete forms will be returned. Completed documents may be submitted in person or emailed to brighterlearning@gmail.com.

• Emergency Form 

• Parent’s Guide to Regulated Child Care Pamphlet Receipt  (Please sign and date at the top)

• All About Me Form 

• Topical Basic Care Product Application Authorization (sunscreen, bug spray, diaper cream, etc.)

   

The following forms must be completed by a licensed health practitioner: 

• Health Inventory

• Immunization Certificate

• Blood Lead Testing Certificate 

• Medication Administration Form 

• Allergy Action Plan  

• Asthma Action Plan
